Client Responsibility for Company Policies

This policy applies to all clients. This policy ensures that all clients are aware of the company's expectations, guidelines, and standards of conduct.

Understanding and adhering to company policies helps maintain a safe and productive treatment room environment, promotes fairness, and ensures compliance with legal and regulatory requirements.

Access to Policies: Company policies and procedures are accessible on company website at www.sugarcookiewax.com or can be requested in hard copy at our suite.

Review and Understanding: Clients are expected to review all policies, forms, FAQs and notices. If any policy is unclear, it is the client's responsibility to seek clarification from a Sugar Cookie Wax esthetician.

Updates and Amendments: The company reserves the right to modify or amend policies at any time, with or without notice. Clients will be notified of significant changes, and it remains their responsibility to stay informed of the latest policies.

Consequences of Non-Compliance: Failure to comply with company policies and procedures, including those outlined in this policy, may result in consequences up to and including fee charges, rescheduled or cancelled appointments, refusal of service, banning from premises, and potential legal action.

Client Acknowledgement:

I, the undersigned Client of Sugar Cookie Wax, LLC acknowledges receipt, review, and understanding of the Sugar Cookie Wax, LLC's  Policies, Notices, FAQs and Forms,  either online, in print and/or verbally and agree to comply with them in their entirety.

I understand the responsibility to seek clarification on any unclear policy, notice or form and have been given opportunity to do so

I understand that Sugar Cookie Wax policies, FAQs, Notices and Forms  are subject to updates, and it is their responsibility to stay informed of these changes.

I agree Sugar Cookie Wax, LLC and its founders, officers, and employees have the right to change recommendation of products and/or services upon in person physical exam and review of client intake information, up to and including refusing service if contraindications are identified.

I agree to waive my right to bring any claim or action, hold harmless, and release Sugar Cookie Wax, LLC, its members, founders, officers, and employees or heirs from any and all liability that may result from care, including but not limited to disability or personal injury arising from the advice, treatment, table accommodation, or for any outcomes or consequences resulting from the above services.
